Occupational Therapy Degree Apprenticeship

This form will be completed every 12-weeks to review your apprentices progress throughout their apprenticeship. Please complete your section before the tripartite date and send this through to hsccolapp@essex.ac.uk prior to the tripartite meeting. On the form you will be asked to consider examples of when your apprentice has demonstrated British Values, Safeguarding, maths and English within their practice. Also, this is an opportunity to praise the progress of your apprentice as well as set targets for development. Tripartite meeting will last an hour and cannot go ahead if the employer, university representative or apprentice is unable to attend. Please prioritise these meetings in your diary.
